Attention Government (LMBR) Schools: Membership can only be accepted from LMBR schools providing pre-payment purchase order number. Pre-payment PO Number
Please fax or email this form to TEA in addition to submitting to shared services for payment. Invoice date for LMBR schools is 24 January 2017
